Scranton, PA – Serving Seniors Inc. is proud to announce that its Patient Advocate Program has been awarded $5,000 Community Needs Grant from the Scranton Area Community Foundation in the Spring 2025 cycle. This vital support will enable Serving Seniors Inc. to continue its mission of providing personalized advocacy and support for older adults in Northeastern Pennsylvania.
The Patient Advocate Program addresses critical needs for seniors being discharged from local hospitals, assisting them in navigating complex healthcare systems, accessing resources, and helping them recover at home and meet their discharge goals. The program has been instrumental in improving health outcomes and reducing stress for seniors and their families in Lackawanna County.
